Perioperative Services - Pre-admission Information

Prior to your planned theatre admission, please follow the below instructions.

Please complete the pre-admission form (ensuring that there are two contact names and telephone numbers included) and return it seven days prior to your anticipated admission date, via one of the options below:

Option 1 - Electronically (Our Preferred Option)

Option 2 - Via post.
You can print the pre-admission form hand write and then post to:

Option 3 - In person at the main reception area of the hospital, 45-63 Coster Street Benalla.

Confirmation of admission time: Nursing staff will contact you 1 day prior to your scheduled theatre with your full admission details including date, time and fasting details if required.
